- To add, change, or remove a name, all parties must sign the Account Status Change Form and provide a current valid photo ID or applicable documentation.
Disconnect Your Service
Please allow at least one (1) business day to process a disconnection of service request.
To disconnect your service, all customers are required to submit a completed and signed Account Status Change Form. Disconnection of service is not accepted verbally over the phone. The disconnection of service must be authorized by all account holders (s).
To return the completed and signed Account Status Change Form, please fax, email, mail, or come to our office. Contact information is available on our Contact Us page.
